GENERAL TERMS & CONDITIONS AND CANCELLATION POLICY FOR REGISTRATION

1. Registration procedure
All registrations must be made by using the official registration form only (or a copy - one form per participant) or by using this registration form on the Internet. No registration form is accepted unless accompanied by full payment.

2. Registration through Internet
When registering through the internet your personal and payment details are secured. All data are transferred by the latest SSL encrypting techniques available (Secure Socket Layer, 128-bit Super Certificate with a 1024-bit key).

3. Confirmation
Your registration will only be confirmed after receipt of the required full payment. In order to avoid any confusion due to mail or bank delays, advance registrations will close on 15 September, 2010. After this date registrations can be made on site at the meeting only from 26 to 30 September 2010 at the then applicable rate.

Delegates are entitled to:
- participate in all scientific sessions;
- attend the Opening Ceremony and Welcome Reception;
- access to the exhibition hall.

Accompanying persons are entitled to:
- attend the Opening Ceremony and Welcome Reception;
- a half-day guided city tour "Getting to know much about Seoul";
- access to the exhibition hall.

4. Liability
In registering for the congress the participants agree that neither the ISSM, the ISSM Executive Office, MECI International nor the Organizing Committee or any of their officers, agents, employees or representatives assume any liability whatsoever.

5. Group Registration
In case of 10 or more participants you can contact the ISSM Executive Office (office@issm2010.info) to obtain the group registration form.

6. Insurance
Participants are strongly advised to arrange their own travel-, health- and cancellation insurance in the unfortunate case of having to cancel the congress participation and/or travel and/or hotelaccommodation because of illness or other (personal) circumstances. Please consult your insurance broker.

7. Terms of Payment
All registrations must be paid in Euros (€).

By credit card
VISA, American Express and Eurocard / Mastercard are the only credit cards accepted for payments.

IMPORTANT: clearly state YOUR name on payment and payment must be free of charges for ISSM.

Please note: a copy of the bank transfer order must be sent (fax or e-mail) to the ISSM Executive Office together with the registration form.

8. Multiple Payments
Multiplepayments must always be accompanied by a list of names and details of payment for each person. The payment must be free of charges for ISSM.

9. Pending Amounts
Any differences (appearing from incorrect application) of registration fees will be collected during the congress.

10. Payment On-Site
On-site payments must be made in Euros in cash or by one of the indicated creditcards.

Name Substitutions
Name substitutions are allowed free-of-charge until 1 September 2010. Name substitutions received after 1 September 2010 will be considered as cancellations and will be processed as new (on site) registrations and are subject to the general cancellation clauses.

12. Reimbursement of Registration Fees
When the cancellation is notified:
- before 1 May 2010: EUR 50 administration costs will be withheld
- between 1 May – 1 September 2010: 50% refund
- after 1 September 2010: sorry, no refund possible.

13. Refunds
All refunds / reimbursements will be processed after the congress.

14. Copy of Registration Form
Keep a copy of this registration form (both sides) for your personal file.
